How to Complete a DUI Evaluation near You

  • Register for a secure account online, call us at 1-800-671-8589,  or register at one of our kiosk locations
  • Complete the DUI assessment questionnaire online
  • Complete a 45- to 55-minute interview with one of our professional evaluators via phone or online video call
  • Receive your report by email to review and approve before forwarding or releasing to authorities (typically within five business days—rush service available)

*Note:If you need to provide us with important documents from the court you may email, fax, or upload them to your account at any time
