How to Request Your Workplace Drug and Alcohol Evaluation
- Register for a secure account online, call us at 1-800-671-8589, or register at one of our kiosk locations
- Complete the initial questionnaire online
- Complete a 45- to 55-minute interview with one of our professional evaluators via phone or online video call
- Receive your report by email to review and approve before forwarding or releasing to authorities (typically within five business days—rush service available)
*Note:If you are a Department of Transportation (DoT) employee, guidelines require you to complete an in-person evaluation only.
