How to Request Your Driver’s License Reinstatement Assessment
- Register for a secure account online, call us at 1-800-671-8589, or visit one of our convenient kiosk locations to register
- Complete the initial questionnaire online
- Complete a 45- to 55-minute interview with one of our professional evaluators via phone or online video call
- Forward any supporting documents (such as any forms required by the DMV or court) to your evaluator
- Receive your report by email to review and approve before forwarding or releasing to authorities (typically within five business days once your evaluator has received all documents—rush service is available)
*Note:Some assessments may require that your evaluator review paperwork from the court or make a brief telephone contact with a character reference on your behalf.
