The goal of an alcohol assessment is to determine the nature of your alcohol use and if you would benefit from education or treatment. You may be required to complete this type of evaluation in one of the following situations:
- Legal charges involving alcohol, such as public intoxication, disorderly conduct, etc.
- Workplace incidents involving alcohol
- Request from a physician or treatment facility
- Self-referred or referred by a concerned family member

How to Obtain an Alcohol Assessment with Us
- Register for a secure account online, call us at 1-800-671-8589, or register at one of our kiosk locations
- Complete the initial basic alcohol assessment questionnaire online
- Complete a 45- to 55-minute interview with one of our professional evaluators via phone or online video call
- Receive your report by email to review and approve before forwarding or releasing to authorities (typically within five business days—rush service available)
*Note: If you need to provide us with important documents from the court you may email, fax, or upload them to your account at any time.
