Steps to Request Your Driver’s License Reinstatement Evaluation

  • Register for a secure account online, call us at 1-800-671-8589, or visit one of our convenient kiosk locations to register

  • Complete the initial questionnaire online

  • Complete a 45- to 55-minute interview with one of our professional evaluators via phone or online video call

  • Forward any supporting documents (such as any forms required by the DMV or court) to your evaluator

  • Receive your report by email to review and approve before forwarding or releasing it to authorities (typically within five business days once your evaluator has received all documents—rush service is available)

*Note: Some assessments may require that your evaluator review paperwork from the court or make brief telephone contact with a character reference on your behalf.
