The goal of an alcohol assessment is to determine whether a clinical alcohol abuse disorder is present and whether treatment could help. You may be required to complete this type of evaluation in one of the following situations:

How to Obtain an Alcohol Assessment for Court

  • Register for a secure account account online, call us at 1-800-671-8589, or register at one of our kiosk locations

  • Complete the basic alcohol assessment questionnaire online

  • Complete a 45- to 55-minute interview with one of our professional evaluators via phone or online video call

  • Receive your report by email to review and approve before forwarding or releasing it to authorities (typically within five business days—rush service available)

*Note: If you need to provide us with important documents from the court you may email, fax, or upload them to your account at any time.
